Vision
If you have vision correction needs that aren’t covered by your insurance or Medicaid or Medicare, please download our Vision Assistance Request Form. Fill it out and give to any Fairbury Lions Club member. If you don’t know any Fairbury Lions Club members, please call 815-419-5087 to make arrangements.
Hearing
In Illinois, health coverage insurers and Medicaid are required to offer coverage for hearing aids for children under the age of 18 years and for adults aged 65+.
